What Is Gynecomastia?

Gynecomastia is the growth of breast tissue in men. It is not just fat: there is a real gland under the nipple, and that is why it does not go away with diet or exercise, no matter how much you train.

The surgery consists of removing that gland and, when needed, the fat around it, to give the chest back a masculine contour. I work through a small incision at the edge of the areola, where the scar hides in the change of skin color. In some cases, when there is excess skin, the technique is a little more extensive.

Your Process: 4 Stages of Your Surgery

Consultation and Study

It all starts with a calm, discreet conversation. I examine the chest, determine how much is gland and how much is fat, review your medical history and the medications you take, and order tests if the case calls for it.

Pre-Surgical Preparation

The internist evaluates you and orders the tests that apply, and the anesthesiologist reviews your case. I give you written instructions for the days beforehand and for stopping medications.

The Surgery

I remove the gland through an incision at the edge of the areola and, when it is needed, I complement it with liposuction of the fat that surrounds the area so that the contour is even and without steps.

Follow-Up Visits and Monitoring

I accompany you at scheduled check-ups to review how you’re healing and to follow how your contour evolves until the result is settled.

What to Expect from Your Gynecomastia Surgery: Day by Day

First days Compression Garment and Rest

You go home with a compression garment or vest that helps your skin adhere to the new contour. The discomfort is moderate and is well managed with the pain medication I prescribe.

First weeks Back to Your Routine

Most patients return to office work within a few days. I ask you to avoid lifting weight and doing chest and arm exercise until I clear you at your follow-up.

First Months — The Contour Becomes Defined

The swelling subsides gradually and the breast takes on its real shape. There may be temporary firmness under the nipple, which is a normal part of internal healing.

The Months After — Final Result

The contour is defined and the scar at the edge of the areola lightens until it becomes hard to make out. If the cause is under control, the result is permanent.

Post-Op Care Specific
to Gynecomastia

Wearing the compression garment and being patient with the swelling are what most influence the final result. These are my instructions:

  • Wear your compression garment or vest as I tell you. That is what makes your skin adhere properly to the new contour.
  • No chest or arm exercise. Until I clear you at your follow-up. Weights are the last thing you go back to.
  • Sleep on your back. For the first few weeks, so you do not press on the operated area.
  • Walk from day one. Short, frequent walks to prevent thrombosis.
  • Protect the scar from the sun. Strict protection for months, especially if you go to the pool or the beach.
  • Don’t be alarmed by the firmness under the nipple. It is internal healing and it softens with time. If it worries you, we check it.
  • No smoking. It compromises healing and how well the skin adheres.
  • Let me know if anything worries you. Increasing pain, fever, sudden swelling on one side, or drainage: call me.

What to Expect from the Result of Your Gynecomastia Surgery: A Masculine Chest

The goal is simple and very concrete: that the chest has a flat, masculine contour, and that you can take your shirt off without thinking twice. It isn’t a transformation surgery, it is a specific correction. Many of my patients have spent years avoiding the beach, the pool or the gym because of this. The change is usually bigger in what you allow yourself to do than in what can be seen.

What shows isn’t the surgery: it is that you stopped hiding.

Permanent Result

The gland I remove does not grow back, as long as the cause behind it is under control.

Discreet Scar

It runs along the edge of the areola, where it blends into the change in skin color. Over time it becomes hard to distinguish.

Even Contour

By removing gland and fat in the same surgery, the chest is left flat, with no steps or dents around the nipple.

What Surgery Does Not Do

It does not correct generalized excess weight and it does not replace exercise, and on its own it does not solve an active hormonal cause. If there is something underlying, that has to be treated too.

Frequently Asked Questions about Gynecomastia

That is a very fair question, and the honest answer is: it depends on your case, on the degree of gynecomastia and on whether skin has to be removed in addition to gland and fat. Under the guidelines of the Colombian Society of Plastic Surgery, prices are not published online. At your consultation you receive a clear, written estimate.

There are several causes. The most common is the hormonal imbalance of adolescence, which often resolves on its own. In adults it can be related to certain medications, to the use of substances such as anabolic steroids, alcohol or marijuana, to being overweight or to an underlying medical condition. That is why I ask so much at your consultation: it is not curiosity, it is part of the diagnosis.

Not always, but I ask for it when the gynecomastia appears quickly, when it is painful, when it is very asymmetric, or when there is no obvious cause. If something active is behind it, I prefer to correct that before operating; otherwise the tissue can grow back.

Yes, as long as the cause is under control. The gland I remove does not grow back. But if you keep using what caused it —anabolic steroids, for example— or if there’s an untreated hormonal condition, the tissue can come back. We talk about that frankly before operating.

No, and this is the frustration almost all of my patients arrive with. Exercise reduces fat, but the gland does not respond to diet or training: it is real breast tissue. In fact, as you lose weight and define your chest, the lump behind the nipple often becomes more noticeable.
