What Is Buccal Fat Removal?

Buccal fat removal is the surgery in which I partially remove the buccal fat pads: pockets of fat we all have between the cheek and the masseter muscle. When they are large, they give the face a rounded look that does not change no matter how much you diet or exercise.

By reducing them, the cheek loses volume and the cheekbone and the jawline gain definition. I work from inside the mouth, so no visible scar is left on your face. I remove only a part: taking out too much ages the face over the years.

Consultation for Your Face

It all starts with a calm conversation. I review the proportion of your facial thirds, the actual amount of fat you have and how your face is going to age. Here we decide whether buccal fat removal adds to you or takes away from you.

Pre-Surgical Preparation

I give you written instructions for the days beforehand and I review your health history and the medications you take. We also talk about oral hygiene, which matters because I work from inside your mouth.

The Surgery

I make a small incision on the inner side of each cheek, identify the buccal fat pad and remove only the portion that fits your case. I close with stitches that dissolve on their own.

Follow-Up Visits and Monitoring

Your cheeks look more swollen than before surgery. That is what we expect and it is unsettling for everyone, so I warn you about it from the consultation: this is not the result.

It goes down gradually and your face starts to look like yours again. A soft diet and the mouth rinses are key during these days.

Post-Op Care Specific
to Buccal Fat Removal

Since I work from inside the mouth, almost all the aftercare has to do with eating and hygiene. These are my instructions:

  • Soft, cold diet for the first few days. Nothing hot, nothing hard, and nothing that takes strong chewing.
  • Rinse after every meal. With the antiseptic I prescribe, to take care of the incision inside your mouth.
  • Ice on your cheeks. For the first 24 to 48 hours, in short stretches, to control the swelling.
  • Sleep with your head elevated. It helps the swelling go down faster.
  • No cigarettes and no alcohol. They slow the healing of the internal wound.
  • Avoid intense exercise. During the first few days, until I clear you at your follow-up.
  • Don’t touch the incision with your tongue. It is tempting, but it can open it or cause an infection.
  • Let me know if anything worries you. Increasing pain, fever, drainage, or trouble opening your mouth: call me.

A good buccal fat removal result is discreet. It slims the middle third and lets the cheekbone and the jaw show more, but it doesn’t change your face or turn you into someone else. If what you expect is a radical transformation, this isn’t the surgery. And if the goal is a very hollow face, I’m going to tell you no: that doesn’t age well.

That is a very fair question, and the honest answer is: it depends on your case, on the type of anesthesia and on whether it is combined with another procedure. Under the guidelines of the Colombian Society of Plastic Surgery, prices are not published online. At your consultation you receive a clear, written estimate.

You are if you have full cheeks from real fat volume, a rather round face and good underlying bone structure. You are not if your face is already thin or long: in that case, removing fat will age you over the years. That is exactly what I assess at your consultation, and I say no often.

More than pain, what you feel is swelling and discomfort when chewing during the first few days, which is well managed with the pain reliever I prescribe. What bothers my patients most is not the pain but seeing their cheeks more swollen than before during the first few days.

Not right away. The slimming becomes visible as the swelling goes down, and the final result takes several weeks to settle. It is a surgery that asks for patience, and I warn you about that from the consultation so you do not get anxious along the way.

No. The fat I remove does not grow back, and that is why it is so important not to take out too much. If over the years your face loses volume naturally, it can be compensated with hyaluronic acid fillers or with a fat graft, but it is much better not to have overdone it from the start.
